Sciatica is a condition characterized by radiating pain from the lower back to the buttocks, thigh, leg, and/or foot. This discomfort can be severe and disabling.  Many people have even gone to the emergency room because of pain.  However, sciatica can be routinely managed through non-invasive means, allowing the body to heal naturally. Dr. Michael Pierce, DC, at Santa Clara, California’s Wellness Revolution Health Center, provides our community with treatments that address the specific cause of sciatica and restore the body’s balance.  He teaches his patients how to live in a way that reduces sciatica’s recurrence.

Why Sciatica Develops

There are numerous causes of sciatic-type pain. 

The most common cause seen by Dr. Pierce is contraction of the piriformis muscle causing compression to the sciatic nerve.  This cause is simply treated by restoring the nerve function to the piriformis via adjusting the lumbar spine, then stretching the piriformis muscle.

The second most common cause is referred pain from the gluteal muscles.  Trigger points in the gluteal medius, gluteus minimus, or tensor fascia lata muscles can produce pain down the outside of the leg to the ankle.  Treatment for this is resetting the nerves causing contraction of these muscles, muscle therapy to the trigger points, and stretching.  Occasionally, core strengthening is required.  Treatment is straightforward.

The third most common cause seen in the clinic is ‘sclerotomal’ pain resulting from disc irritation (without herniation).  [Sclerotome refers to the part of the body that grows from the embryonic vertebrae and discs, which are the first system developed in the embryo.] This disc irritation can be from spinal misalignment, degeneration, or injury, and can directly refer pain down the buttocks and leg.  Treatment for sclerotmal pain requires precise chiropractic care and often nutrients to speed the disc healing.  Often, we use laser therapy to reduce the pain.  Relief and stabilization are straightforward, but take a bit longer than the first two causes.

The fourth cause is a disc bulge or herniation compressing the nerve roots at the spine that comprise the sciatic nerve.  This pain is often worse with sitting or movement.  About ninety percent of those with disc issues get full and lasting relief.  This treatment responds best to non-force chiropractic treatment, laser, and medicated patches that speed the healing of the disc. Only about one in twenty people need to be referred for medical intervention.

While chiropractic care to realign the joints and stretch the muscles plays a crucial role in treating sciatica, making certain lifestyle adjustments can improve recovery and prevent future flare-ups.

One of the most helpful lifestyle changes for managing sciatica is making a routine of regular exercise. Low-impact activities such as swimming or cycling strengthen the muscles that support the spine and improve flexibility. Stretching exercises, particularly those targeting the lower back and hamstrings, can alleviate tension and reduce pressure on the sciatic nerve. Maintaining a Healthy Weight

Excess weight can place additional stress on the spine and exacerbate sciatica symptoms. Maintaining a healthy weight through a balanced diet and regular physical activity is essential for reducing pressure on the sciatic nerve. Practicing Good Posture

When sitting, ensure that your back is supported, your feet are flat on the floor, and your knees are at a right angle. This could require you to change the setup of your workstation. When lifting heavy objects, use your legs for support rather than bending at the waist.

Stress can exacerbate pain, making it essential to practice stress management techniques frequently. Mindfulness, meditation, and deep breathing exercises can help reduce stress levels and promote relaxation. Additionally, setting yourself up for more restful sleep by avoiding electronic screens an hour before bed and controlling the moisture level in your bedroom can improve your overall well-being and help manage sciatica pain.
